Below are the steps you need to take to become our newest client. Don't worry; we keep things simple.
Book Your Appointment
Call or reserve your appointment online to secure your date to come in for your initial visit.
Bring the Right Gear
Make sure that you are wearing comfortable and flexible clothing, and close-toed shoes for your appointment. Athletic attire is recommended.
Coverage & ID Documents
Be sure to provide us with your insurance information in advance or an alternative form of payment. We also require your driver's license, state ID, or passport for our records.
Referrals or prescriptions are not required, but if you have one, please bring it as well.

Medical History
We conduct a thorough review of your medical history, diagnostic tests and any recent events that may have contributed to your current condition.

Examination
Your evaluation will include AN assessment of your current functional deficits, pain level and posture, as well as a thorough evaluation of your flexibility, strength, balance and endurance.

Plan of Care
Following your evaluation, your physical therapist will thoroughly explain your personalized treatment plan and expectations for recovery, as well as provide a commitment letter to you.
